Incident Report Request

DFES records information about emergency incidents that we attend.  We can provide a copy of our Incident Report to you in specific circumstances.  We can only do this:

  • After the incident is resolved, crews have left the scene and completed their reports
  • It is a Residential Property (i.e. Home)
  • You can prove that you are the Owner or Occupant

Any evidence you provide must only be sent using this secure portal.  Your documentation will be deleted once verified.
